These are the practice's current standard self-pay, time-of-service fees for common professional services. Insurance, workers' compensation, and employer-direct arrangements require verification or authorization and may use different contracted rates. Surgical and other procedural charges are estimated individually in writing.
These prices apply only after the practice confirms a self-pay arrangement. They do not apply to insurance, workers' compensation, or employer contracts unless agreed in writing. Procedures, injections, braces, supplies, extensive record review, formal reports, and third-party charges are separate. Do not pay until the practice confirms the service or sends an invoice.
